Contest Form
Once you have joined Amazon Prime or purchased an Amazon Kindle, you need to
complete the form below in order to be entered into the contest. This form must
be filled by November 15th 2008.
The Fine Print:
- Contest open to all legal USA residents, 21 years of age or older. Void where
prohibited.
- Drawing will be for a $1000 Amazon Gift certificate and will only occur if at
least 200 confirmed subscriptions/purchases occur between August 22nd 2008 and
October 31st 2008.
- Winner will be determined based in a random drawing which will occur on November
29th 2008 and gift certificate will be issued within the next 14 days after
drawing. Winner will be notified via email.
